I have created this form to collect shirt sizes for our team. The last day to submit your response is 4/26. If no response is received, no shirt will be ordered. Please submit a response for EVERY swimmer on the team. 

15+ swimmers will still need to purchase a shirt from me the following two Thursdays to have their shirt order placed.
